Incident Form - Illness and Injury

Player health and well-being is the foremost priority for Waikato Water Polo and while we aim to minimize or prevent incidents, we understand that given the physical nature of the sport incidents involving injury or illness can arise. When this happens, we ask that members of the community please take the time to provide details of the incident so that we can repsond to these in a manner which places player safety at the forefront to ensure any injured parties are well-informed and cared for, with follow-up plans and guidance in place to ensure them the best of recoveries and/or to minimise and prevent future incidents from occuring.
